Professor Stephens was my site evaluator for my emergency medicine rotation. During our site evaluation, I presented a patient, provided an article related to the case and review my pharm cards. Although by now I have written countless H&Ps, there are still things I can improve on. One thing that came to my attention is how H&Ps differ based on the setting in which the information is obtained. In emergency medicine the physical exam portion is often not as extensive as one in internal medicine or family medicine. Being able to adjust the H&P to the appropriate setting is something I need to work on. One benefit to having professor Stephens as my site evaluator for a second time is that there wasn’t any guessing as to what she required. With the same person grading my H&Ps, I am able to more accurately track my progress. With the thorough feedback that she provides, I hope to continue improving my patient documentation.
